Antivenom availability – crisis or not?

Southern Africa has well over 170 different kinds of snakes and our fair share of highly venomous snakes. While most of them are not medically important, snakes like the Black Mamba, Cape Cobra, Mozambique Spitting Cobra and Puff Adder, to mention but a few, are highly venomous and a bite from one of these snakes could be life-threatening.
The only effective treatment for a serious snakebite is antivenom, however not all snakebite victims require antivenom – in fact only about one out of ten snakebite victims that are hospitalised need and receive antivenom. This is because of the level of envenomation. You could easily get a bite from a Mozambique Spitting Cobra with no symptoms whatsoever. We refer to such bites as dry bites – the snake bit without injecting venom. Or you may have mild symptoms – a bit of swelling and no worse than that. Or, of course, one can receive a serious bite with serious consequences. For this reason, doctors in hospitals treat symptoms and not snakebites.
Antivenom for snakebites has been around for more than 100 years and in South Africa the South African Vaccine Producers, a division of our National Health Laboratories, produce antivenom for spiders, scorpions and snakes. For snake envenomation we have a monovalent Boomslang antivenom as well as a polyvalent antivenom that is made for the venom of ten snake species, including the mambas, cobras, Rinkhals, Puff Adder and Gaboon Adder.
The South African Vaccine Producers has run into production problems since Covid and currently have no stock of antivenom. They are busy with a revamp of their facilities and hope to have antivenom available in April 2025.
With these shortages, several overseas companies have started importing various antivenoms into South Africa and as these drugs are not registered in South Africa, they are brought in under Section 21 of SAPHRA regulations. For any doctor to obtain or use the imported antivenoms, they need to do quite a bit of paperwork, and one can safely say the imported antivenoms are not that freely available. This also applies to veterinarians.
